How do you play ladders?

Hi, welcome to this totally non-controversial thread! I'm trying to capture some info on how people play ladders in the real world. So when you answer the questions below, please don't check the rules first. Just answer with your gut response, based on how you have played in the past or how you'd guess if you were asked at a tournament. Don't worry about whether your answers are self-consistent either. Just take each one as it comes.
