Can somebody give some clarification on how the ladders work exactly? Do you just put the +1 or +3 circle over top of your figure?

Yeah, it's just a visual way to denote which rung of the ladder a figure is on.

EDIT: note that the circles are semi-transparent, so if you bring them to the front, they can easily be laid over the unit in question.
