Well if he is already wounded by some form of attack, his body is already undergoing stress so the added stress from using his powers could lead to a heart attack. Seems like the chance to wound himself should be tied to his power use though in that case. Lazy has sold me on the idea of having him randomly suffering from a heart attack, so I think I would do both. Give him less than 4 life, since he is not a fighter and in very poor health to start with, then give him an 'after using this power, roll an unblockable attack die' power to boot.

And this guy will get owned by squads, which I'm not sure should happen. My issue, how does stress or anxiety causing a heart attack relate to an additional wound after an attack?

It isn't an additional wound after an attack - it's 50/50 chance of a wound at the end of the round if he's already wounded, and an already established power for representing a character with a pre-existing medical condition on Speedy (Mia Dearden), who is awesome and doesn't actually die too quickly as long as you keep her decently protected.

Ok what if we did this

MASS INCREASE
Any time an opponent would begin the movement of a figure within 6 clear sight spaces of Harry Leland, you may first roll the 20-sided die. If you roll an 11 or higher, the figure must reduce 1 from it's movement that turn. You may continue rolling the 20-sided die until 11 or higher is no longer rolled or its movement is reduced to 0. At the end of the round if Harry leland used Mass Increase roll an unblockable attack die against him.

Then we could drop Delicate Health, if we go this route though I would like to keep his life at 4. I feel like with both he's going to drop too early.

I actually like this. It shows where the stress is coming from...

Yeah I like the version tied to mass increase, though it needs to still be a separate power or the combined power needs a less generic name. Just flat delicate health didn't feel quite right, being tubby isn't really on the same level as having AIDS or cancer or whatever.

Really @Soundwarp SG-1 ? I'm "tubby" and I hardly think that equates to Cancer, HIV, AIDS or any other life threatening disease. None of those other people can really help themselves with diet and exercise but I can and have since my car accident in 2016.

I made some updates to my Manphibian design. I realize that Abe Sapien had a power that I was essentially going for already. Any feedback?

Manphibian also has the class of Creature. Manphibian does not stop his movement when entering a water space.

CLAW ASSAULT
When rolling attack dice against a small or medium figure without the Super Strength special power, if Manphibian rolls a skull or blank on every die, the defending figure cannot roll any defense dice.

SUBMERGE 12
If Manphibian is attacked while on a water space, you may roll the 20-sided die. If you roll 12 or higher, Manphibian takes no damage and you may place him on any unoccupied water space within 5 spaces of Manphibian that is no more than 3 levels above or below Manphibian's base. If Manphibian is placed adjacent to the attacking figure, you may roll an unblockable attack die against that figure. When Manphibian moves with Submerge, he will not take any leaving engagement attacks.

What's the rationale on dual-classing him? If it's one of those Howling Commandos books, IMO, you'd need to implement those kind of teams via some central leader figure turning Creatures into Agents anyway, so I don't think I'd dual-class him for that (unless he has Agent history outside those books).
