If he uses the first option and rolls 2+ skulls, and hasn't exhausted his max of 3 attacks in a single turn, then he can at any point switch to the 2nd option. Think of the 1st option as a combo extender, and the 2nd option as the combo ender, because he can't attack again following the 2nd option. So with each attack is a gamble. You decide if you want to try and extend it, or finish it there. If your first attack you roll 2+ skulls, then your 2nd attack you try to do the first option again and don't get 2+ skulls, his attacks are done. So yes, it is intended that you decide if you want to risk extending the combo or not, or if you just want to use the ender.
