1) Logically it doesn't make a whole lot of sense, but going by the rules I'd say yes, they are affected. I don't see anything in the rules to suggest otherwise.

2) I'd say yes to both of these questions as well.

3) We've given height bonuses when attacking the door to a higher figure. We've also given the door a defense boost when its higher than the attacking figure.
