Dig the change to just preventing him from attacking instead of negating the power and removing OMs.

I would actually do this, trimming a bit more. (He doesn't have an SA, so I don't think we necessarily need to say normally every time.)

RAGE AND REMORSE
After attacking normally with Spider-Man, roll the 20-sided die. Subtract 1 from the roll for each attack made this turn. If you roll 5 or less, Spider-Man must attack again. If you roll 6-10, if Spider-Man can attack an opponent's figure, he must attack again. If Spider-Man destroys a figure with his normal attack, he may not attack again this round.

I think the must makes the "6-10" into a "negative" effect, which justifies the low roll. But I think all of the extra attacks being a "bad thing" feeds into the "slipping into a rage" narrative you're trying to build here. If you roll bad, he goes too far. If you roll REALLY bad, he goes too far and might also accidentally slap his wife or something.

   Yes. This is absolutely one to watch. The D20 can be upped as needed, though, and requires you to hit a 9+ rolls and then a 50/50 die roll, so it’s not automatic at all.
3. Does the design immobilize figures or prevent turn-taking (Blob, Spider-Girl, Diablo, Green Lantern Alan)?
   No.
4. Does the design move opponent's figures in a way that's easy to pull off and can move figures multiple spaces in multiple directions (Graviton, Magneto, Vulture)?
   No.
